We’re all patients at some point, and as we work towards a health and care model that’s focused on keeping us well and out of hospital, we need to turn our attention to engaging everyone to take an active role in improving and maintaining health and wellness. The explosion of self-management apps and personal monitoring devices can help us to create a single source of truth as we look to move from reactive care to proactive, coordinated health.

Boosting patient engagement

Our Patient Portal allows for a two-way conversation between individuals and their care providers. It means that citizens can experience more active participation in their own care, enabling them and their care teams to be aligned, informed and educated on an ongoing basis.

Feel closer with remote monitoring

Technology is everywhere. Nowadays, a care team can prescribe technological devices that offer active, remote monitoring of a person’s health status from the comfort of their home. Using these devices, people are empowered to manage their health at their own convenience. In turn, care providers receive continual, vital information related to the person’s health condition.

A significant part of a person’s life is spent in work, so managing health in that environment makes perfect sense. We work with more than 150 employers, providing population health management services across on-site health, administration, and health and engagement. How will Cerner help clients create an immersive experience for engaging citizens?

Cerner has been extending its HealtheLife portal to support third party content and applications to bring together experiences for citizens into a single portal. We can also provide our citizen-facing solutions into other portals or services. We provide a single sign-on with context sharing to enable personalisation of experience.

Whilst a cohesive patient portal experience is helpful for many, we also recognise that citizens use apps – rather than a web portal – on their mobile devices to manage their day to day activities. Our Patient Portal works as an app on mobiles, and our population health management solutions come with open APIs so we can share the care record or gaps in care with other citizen apps. We are integrating personal device monitoring to ensure that our solutions can make use of the citizens’ data and provide a joined-up experience.

Cerner’s HealtheLife portal provides access to the health record in the hospital or across the population, secure messaging to communicate with care professionals and share information, and questionnaires for pre- and post-visits. It also gives individuals the ability to book appointments, order repeat prescriptions and other features to make their lives easier.

Cerner’s CareAware® device integration platform enables personal monitoring devices – individually or through aggregators like Validic and Apple HealthKit – to integrate citizens’ data into their EHR record. This allows care professionals to see and act on the information, or add into the population health record in HealtheIntent, contributing to the single source of truth longitudinal record and counting towards quality metrics and patient goals.

Cerner provide solutions for workplace wellness including software, health coaching and health navigation services. These solutions address workplace absenteeism and presenteeism, as well as reduce spend on healthcare through addressing risk factors and staff retention rates.

Cerner and our clients approach to population health is to know where to focus attention, to find and engage citizens, and actively manage everyone to improve their health and wellbeing. Our HealtheIntent platform is programmable and acts as an intelligent learning system. We can risk stratify and segment populations to find groups that need focus and attention, and use algorithms to attribute citizens to the right health and care professionals including intelligent selection of the right care manager for the individual. Our care management solutions can track citizens’ self-monitoring results and know when to contact them when situations change, and we are developing more advanced ways to identify which citizens are most impactable and would benefit most from health coaching and care management.
