Helping patients navigate the care continuum requires dedicated work and the latest technology. In these three sessions at Cerner Health Conference 2017, we'll explore how health care organizations can make behavioral and physical health plans unique to each patient.

3. Becoming a Data-Driven Organization on the Journey to HIMSS 7 (Session #744)
Damian Jankowicz, vice president information management, chief information officer and chief privacy officer at the Centre for Addiction and Mental Health
Over the last five years, the Centre for Addiction and Mental Health (CAMH) has transformed itself into a data-driven organization by leveraging data from its Cerner Millennium® system and extensive research enterprise to advance clinical care and brain science. In this session, Damian Jankowicz will discuss strategic and tactical elements required for the transformation. He will also present a number of case studies in which data is used to drive care, including computerized physician order entry, closed loop medication administration and care planning, as well as specialized examples. The transformation is presented in the context of the journey to HIMSS 7.
